Accidents Reporting and Investigation

The effective reporting and subsequent investigation of incidents and injuries allow Geofizyka Toruń to prevent the recurrence of similar incidents or injuries, by improving systems of safe working practice and procedures.
Geofizyka Toruń requires all incidents and injuries to be reported, both for personnel of Geofizyka Toruń and sub-contractors. The Company's Incident Report Form and Incident Investigation / Follow-up form are the formats that are used.
The level of management involvement in injury and incident investigations depends on the potential consequences of the incident or injury. The higher the level of potential consequence is, the higher the level of management is involvement in the investigation.
The Management System Coordinator / Member of the Board will decide the appropriate level of investigation, and the manning of investigation teams for all incidents other than those categorised as level 4. However, the minimum investigation and reporting requirements for incidents involving emergency response are as below:
- To the full requirements of the national authorities of the area of operation.
- In accordance with the policy of the Client (after consultation).
The incident categories detailed on the following page have been established to facilitate an adequate and appropriate response:

All accidents and dangerous events at work are notified and recorded to management. For aims of statistics there are prepared reports with the following data: employment, quantity of accident at work with partition on deadly events and quantity of lost time injuries. Substantial and frequency coefficients of accidents at work are also calculated. Accidents at work and dangerous events are registered with partition according to needs (monthly, yearly, etc.,).
In the range of reporting and investigation of accidents at work, Decree of Cabinet of Ministry dated on 28. 07. 1998 (determination of circumstances and reasons of accidents at work and manner of their documentation and also range of information placed in register of accidents at work) is obligatory.
In case of accident at work, appointed team determines reasons and circumstances of the event, prepares records, analyses its reasons and introduces conclusions to be realised as to avoid similar events occurrence. Circumstances, reasons of accidents and also manner, of how such events should be prevented are talked over with workers. Small incidents, which occurred during work, have to be notified to the supervisors, who discuss them over with personnel on meetings.
